Go Paperless - Sign Up for Electronic 1095-C Delivery

For the first time, UC employees may now opt-in to receive electronic 1095-C forms via the UCPath online portal.

Going paperless is easy! Here’s how:

Employees can log into UCPath and follow the navigation path: Employee Actions > Income and Taxes > Online 1095-C Consent; then, select the consent check box and submit. Employees may also go paperless for Form W-2 and direct deposit.

2023 sign-up deadlines:

Employees must sign up before January 1, 2023, to receive their 2022 1095-C electronically and before January 13, 2023, to receive their 2022 W-2 statement electronically.

What is the 1095-C form?

The 1095-C form confirms that an employee has health insurance; it is a reporting document required by the Affordable Care Act (ACA). Many employees store the document and present it to their tax professionals annually.

Data security reminder:

UCPath will notify employees in January when electronic tax statements and forms are available. Employees should access 1095-C and W-2 statements directly through UCPath online using a safe, known link.

UC does not send tax statements to employees by email or text. An email or text with a link or an attachment for viewing your tax forms is a phishing scam designed to gain private information. Employees should not open attachments or click on email links that claim to give access to tax forms.
